This page contains information on the C.A.R. MES Vision Plan
OPEN ENROLLMENT for the C.A.R. Medical and Vision plans runs from April 1st through May 15th for a June 1st effective date and from October 15th to November 30th for a January 1st effective date. New C.A.R. members or eligible full time employees of members may enroll between their 60th and 120th day of membership or employment.

- All eligible existing C.A.R. Members and eligible employees may join a Kaiser or Anthem Blue Cross Medical plan, and/or the MES Vision plan during our bi-annual Open Enrollment periods: April-May for a June 1st effective date; October-November for a January 1st effective date.
- All eligible existing C.A.R. Members and eligible employees may join the MetLife dental plan during the Annual Open Enrollment Period: October - November for a January 1 start date.
All eligible members and employees who have been members or employees for at least 60 days may enroll within 30 days of experiencing a "Qualifying Event".

The C.A.R. vision plan is adminstered through Medical Eye Services. If you use a MES provider, you will not need to file a claim since your provider will verify eligibility and submit the claim directly to MES. If you use a provider who is not contracted with MES, you will need to submit the MES Vision claim form after your appointment. To download a copy of the claim form, Click HERE
